EVALUASI PENERAPAN SISTEM KREDENSIAL PERAWAT DALAM PENETAPAN KEWENANGAN KLINIS RUMAH SAKIT DI KOTA MAKASSAR

ABSTRACT

A credential process is an expertise verification series for the clinical staff who comprise the medical staff, nursing staff, and other clinical staff to obtain the clinical authority. The clinical authority discrep ancy with the nursing staff practice area is still found in several hospitals, so that it is necessary to conduct a research to perceive the credential system implementation which has been regulated in PMK 40 Year 2017. The research aims at evaluating the nurses' credential system implementation in the hospital clinical authority determination in Makassar City. The research used the analytic descriptive method. Samples were taken using the probability sampling technique. The total samples were 188 nurses from three hospitals, namely theprovincehospital, cityhospital, andprivatehospital which were in line with the inclusive and exclusive criteria. Data were collected using the nurses' staff credential questionnaire based on PMK Number 40 Year 2017 The research result indicates that the assessment process compatibility with the credential system indicates the compatibility with 138 (73.40%) nurses, the authority determination process in the credential system indicates 131 (69.68%) nurses, and the assignment process based on SPKK indicates the compatibility with 140 (74.47%) nurses. The nurses credential system evaluation in the clinical authority determination of the 3 hospitals which are investigated have generally been compatible with the standard of PMK 40 Year 2017, however, it is necessary to have the synergy of the nursing division and committee in conducting the periodical achievement observation during SPKK period in effect to maximise the credential system as the instrument to improve the nursing quality.

Proses kredensial adalah suatu rangkaian verifikasi keahlian bagi staf klinis yang terdiri atas staf medis, staf keperawatan, dan staf klinis lainnya untuk memperoleh kewenangan klinis. Ketidaksesuaian antara kewenagan klinis dan area praktik staf keperawatan masih ditemukan di beberapa rumah sakit. Oleh karena itu, perlu dilakukan penelitian untuk melihat pelaksanaan sistem kredensial yang diatur dalam PMK No. 40 Tahun 2017. Penelitian ini bertujuan mengevaluasi pelaksanaan sistem kredensial perawat dalam penetapan kewenangan klinis rumah sakit di Kota Makassar. Penelitian ini menggunakan desain penelitian deskriptif analitik. Teknik pengambilan sampel menggunakan probability sampling dengan jumlah sampel 188 orang perawat dari tiga rumah sakit yaitu rumah sakit provinsi, kota dan swasta yang sesuai kriteria inklusi dan eksklusi. Pengumpulan data menggunakan kuesioner yang mengacu pada PMK No. 40 Tahun 2017. Hasil penelitian menunjukkan bahwa terdapat kesesuaian proses assesmen pada sistem kredensial didapatkan sesuai 138 orang (73.40%), untuk proses penetapan kewenangan pada sistem kredensial didapatkan 131 orang (69.68%) dan untuk proses penugasan berdasarkan SPKK didapatkan sesuai 140 orang (74.47%). Evaluasi sistem kredensial perawat dalam penetapan kewenangan klinis dari 3 rumah sakit yang diteliti pada umumnya sudah sesuai standar PMK 40 Tahun 2017 namun demikian, diperlukan adanya sinergi dari bidang keperawatan serta komite dalam menjalankan observasi capaian secara berkala selama periode SPKK berlaku untuk memaksimalkan sistem kredensial sebagai alat meningkatkan mutu keperawatan.
