Abstract (Abstrak)

Background. Prolonged right ventricular pacing (RVP) may have adverse electromechanical effects on the left ventricle (LV). Conduction system pacing such as left bundle branch pacing (LBBP) has emerged as a promising alternative to prevent ventricular dyssynchrony. Objective. To assess the short-term effects of left bundle branch pacing using stylet-driven leads on right ventricular systolic function in patients with sinus node dysfunction and atrioventricular block. Methods. This retrospective cohort study included 59 patients who underwent permanent pacemaker implantation at Dr. Wahidin Sudirohusodo Hospital. Right ventricular systolic function was assessed using TAPSE and S' Lateral parameters before and 6 months after implantation. Results. There were significant improvements in both right ventricular systolic function parameters after 6 months of implantation. TAPSE increased from 2.0 (1.6-2.7) mm to 2.0 (1.7-2.7) mm (p=0.001), and S' Lateral increased from 11 (9-17) cm/s to 12 (1017) cm/s (p<0.001). Subgroup analysis showed no significant differences in right ventricular function changes based on gender, DM status, hypertension, or EF category. Conclusion. Left bundle branch pacing using stylet-driven leads demonstrates beneficial effects on right ventricular systolic function within 6 months after implantation, regardless of patient baseline characteristics.
