Abstract (Abstrak)

Chronic low back pain is a health problem that often causes prolonged pain, functional limitations, and decreased quality of life. This condition is influenced by biomechanical and inflammatory factors. This study aims to analyze the relationship between Lumbosacral Lordotic Angle (LLA), Neutrophil-to-Lymphocyte Ratio (NLR), Monocyte-to-Lymphocyte Ratio (MLR), and erythrocyte sedimentation rate (ESR) with pain levels (Numerical Pain Rating Scale / NPRS) and disability (Oswestry Disability Index / ODI) in patients with chronic low back pain. This study aims to analyze the relationship between LLA and inflammatory parameters with pain and disability in patients with chronic low back pain. Methods: This study is an analytical observational study with a cross-sectional design conducted at Dr. Wahidin Sudirohusodo General Hospital Makassar and network hospitals from October 2025 until the sample size is met. The subjects were chronic low back pain patients aged 20–70 years who met the inclusion and exclusion criteria, with a consecutive sampling technique. The minimum sample size was 24 subjects. The variables studied included the Lumbosacral Lordotic Angle (LLA), inflammatory parameters such as NLR, MLR, and ESR, as well as clinical outcomes in the form of NPRS and ODI values. Data were analyzed using normality tests, comparative tests, and correlation tests according to data distribution with the help of GraphPad Prism v9. A p value <0.05 was considered significant. Results: This study involved 42 subjects, mostly women with normal BMI, active workers, normal lumbosacral curvature, and moderate disability. The mean LLA value was 46.45, NLR 2.34, MLR 0.21, ESR 17.17, and ODI 33.55. LLA was positively and significantly associated with NPRS and ODI, while of the inflammatory parameters, only ESR was significantly associated with NPRS. The cut-off value of LLA >44.8 also showed good predictive ability for moderate-severe pain. Conclusions: Lumbosacral lordotic angle (LLA) is associated with pain intensity and disability in patients with chronic low back pain, whereas of the inflammatory parameters, only ESR is associated with pain intensity. LLA values >44.8 also show good predictive ability for moderate-severe pain
