Abstract (Abstrak)
ARIFUDDIN SIDE. Reduction in Spinal-Induced Postoperative Urine Retention
with Metoclopramid in Lower Extremity Orthopedic Surgery: A Randomized
Double Blind, Placebo-Controlled Study (Supervised by Wahyudi).
Background : Post operative urine retention(POUR) is frequent complication
associated spinal anaesthesia by blocked parasimpathies outonomic nervous
sistem in spinal cord S2-S4. Result not realesed asetilcholine post synaptic.
Smooth muscle of bladder fail to contraction
Objective : We evaluated the effect of metoclopramide as a
parasympathomimetic that inhibited cholinesterase enzyme (PCHE and ACHE) in
vitro and in vivo to post operative urine retention induced spinal anaesthesia in
inferior extremity orthopedic electif surgery.
Methode : Fourty participants scheduled for inferior extremity orthopedic electif
surgery randomly into two groups. Before induction of spinal anaesthesia Group
M (n = 20) received intravenous metoclopramide 20 mg,; Group N (n = 20)
received normal saline. Time first micturation and POUR were assessed.
Result : Time for first micturation significant shorter in Group M 5,41 + 0,56 and
9,93 + 0,65 in Group N (P = 0,000), and had significantly lower incidencesof
POUR in Group M (P = 0,013)
Conclusion : Metoclopramide 20 mg given intravenously before spinal
anaesthesia reduced POUR. Time for first micturation also lower in inferior
extremity orthopedic electif surgery.
