Abstract (Abstrak)

Latar Belakang. Penyakit Ginjal Kronik (PGK) merupakan gangguan fungsi ginjal progresif yang menyebabkan pasien rentan mengalami kelebihan cairan dan komplikasi respirasi. Pada tahap lanjut, pasien PGK membutuhkan terapi pengganti fungsi ginjal yaitu hemodialisis yang dilakukan secara rutin dengan frekuensi 2 kali/minggu atau 3 kali/minggu sesuai kondisi klinis pasien. Peak Expiratory Flow Rate (PEFR) merupakan parameter sederhana untuk menilai kemampuan ekspirasi maksimal sebagai gambaran fungsi ventilasi paru. Perubahan nilai PEFR sebelum dan sesudah hemodialisis dikaji untuk memahami apakah frekuensi hemodialisis berperan dalam mempertahankan atau mengubah kemampuan ekspirasi pasien. Metode. Desain penelitian Pre-Experimental menggunakan rancangan Two Group Pre-Post Test Design dengan pendekatan komparatif untuk membandingkan perubahan variabel pada dua kelompok subjek dengan frekuensi terapi yang berbeda. Hasil. Hasil uji Wilcoxon pada kelompok 2x/minggu menunjukkan nilai p = 0,794 dan hasil uji Paired T-Test pada kelompok 3x/minggu menunjukkan nilai p = 0,315, yang berarti tidak terdapat perbedaan signifikan pada nilai PEFR sebelum dan sesudah hemodialisis pada kedua kelompok. Selanjutnya, hasil uji Independent T-Test menunjukkan nilai p = 0,306, yang menunjukkan tidak terdapat perbedaan signifikan pada perubahan nilai PEFR antara kelompok hemodialisis 2x/minggu dan 3x/minggu. Kesimpulan. Hemodialisis tidak memberikan pengaruh perubahan yang signifikan terhadap nilai PEFR, dan tidak terdapat perbedaan perubahan fungsi ventilasi paru (PEFR) yang signifikan antara kelompok frekuensi hemodialisis 2x/minggu maupun 3x/minggu. Background. Chronic Kidney Disease (CKD) is a progressive kidney dysfunction that makes patients susceptible to fluid overload and respiratory complications. In advanced stages, CKD patients require kidney replacement therapy, namely hemodialysis, which is performed routinely twice or three times a week depending on the patient's clinical condition. Peak Expiratory Flow Rate (PEFR) is a simple parameter to assess maximal expiratory capacity as a proxy for pulmonary ventilation function. Changes in PEFR values before and after hemodialysis are studied to understand whether hemodialysis frequency plays a role in maintaining or changing the patient's expiratory capacity. Purpose. This study aims to determine the differences in PEFR value changes before and after hemodialysis in CKD patients undergoing hemodialysis twice a week and three times a week at Hasanuddin University Hospital, Makassar. Methods. A pre-experimental research design was utilized using a Two Group Pre-Post Test Design with a comparative approach to compare changes in variables across two groups of subjects with different therapy frequencies. Results. The Wilcoxon test results in the twice-a-week group showed a value of p = 0.794, and the Paired T-Test results in the three-times-a-week group showed a value of p = 0.315, indicating no significant difference in PEFR values before and after hemodialysis in both groups. Furthermore, the Independent T-Test results showed a value of p = 0.306, demonstrating no significant difference in the changes of PEFR values between the twice-a-week and three-times-a-week hemodialysis groups. Conclusion. Hemodialysis does not exert a significant effect on acute changes in PEFR values, and there is no significant difference in the changes of pulmonary ventilation function (PEFR) between the twice-a-week and three-times-a-week hemodialysis frequency groups.
