Abstract (Abstrak)
The cholelithiasis is one of the most common gallbladder disorders that accounts nearly ten percent of all gastrointestinal diseases. Several factors have been postulated to be related to this disease and comprised of aging, obesity, and endocrinopathy, such as the thyroid dysfunction. The thyroid dysregulation increases the risk of the cholelithiasis through several mechanisms; it is associated with the impaired cholesterol metabolism and bile secretion. The research aims to evaluate the information of TSH and fT4 level with the number of cholelithiasis cases in the tertiary hospital. This was the cross-sectional descriptive study that was conducted from July 2022 to May 2023. The information was gathered from the eligible medical records that fulfilled the requirements and inclusive criteria such as the patients who were diagnosed to suffer from the cholelithiasis, or those who underwent the thyroid function test (TSH and fT4 level) along with the abdominal ultrasound for gallstone screening. Samples were selected using the consecutive sampling technique. The research result indicates that a total of 102 participants are included in this study, in which 32 of them are positive to have the gallstone based on the USG examination and is predominated by the female population (63.7%). Nearly half of the patients with the positive gallstone finding of USG have the normal TSH ($1.92%) and fT4 level (63.46%) Overall thyroid function analysis reveals that 51.92% patients have the normal thyroid function, 28.84% patients have the subclinical hypothyroidism, 17.3% have the subclinical hyperthyroidism, and clinical hypothyroid is found in I patient (1.92%). The hypothyroidism is associated with sluggish metabolism, so that it causes the disruption in the cholesterol clearance and bile supersaturation that eventually leading to the cholelithiasis. Another mechanism is through impaired sphincter of Oddi relaxation seen in the hypothyroidism along with the diminished gallbladder contraction resulting in the bile stasis. It can be concluded that the thyroid function and cholelithiasis are still needed to be comprehensively evaluated, although the hypothyroidism is frequently seen among the patients with the cholelithiasis.
