Abstract (Abstrak)

Rini Rahman. Analysis of Urine Laminin Level as Early Markers of Diabetic
Nephropathy in Type 2 Diabetes Mellitus Patients (supervised by Liong Boy
Kurniawan and Nurahmi)
Diabetic Nephropathy (DN) is a prime cause of morbidity and
mortality in diabetics and the primary cause of end-stage renal disease
globally. Glomerular basement membrane (GBM) thickening is a hallmark
of DN, and the degree of thickening correlates with albuminuria. Laminin is
a glycoprotein found in GBM, and mesangial plays a role in the
pathophysiology of DN.
This study was aimed to analyze urine laminin level as an early DN
marker in type 2 Diabetes Mellitus patients. This cross-sectional study used
a sample of type 2 diabetes mellitus sufferers. The number of samples in
the study were 73 samples consisted of 33 non-DN samples and 40 DN
samples. Urine Laminin level was examined using the ELISA method. Data
were analyzed statistically using the Mann Whitney, Kruskal Wallis, and
Spearman tests.
The results showed statistically significant differences in urine
laminin levels according to the normoalbuminuria group (270.1±28.5
ng/mL), microalbuminuria group (300.8±21.0 ng/mL), and
macroalbuminuria group (326.8±3.9 ng/mL) with p<0.001. The urine laminin
level in the DN group was higher than the non-DN group (313.8±31.4 ng/mL
VS 270.1±28.5 ng/mL; p <0.001). There was a positive correlation between
albumin and laminin (p <0.001; r=0.692). The cutoff value of Laminin 282.0
ng/mL gave sensitivity and specificity values of 90.0% and 63.6%,
respectively. The positive predictive value and the negative predictive value
were 75.0% and 84.0%, respectively, with an accuracy of 78.1%. Our
conclusion was urine laminin can be suggested as an early DN marker in
type 2 DM patients.
