Abstract (Abstrak)

Background: Pulmonary tuberculosis (pulmonary TB) remains a major public health challenge worldwide, with number of deaths estimated at 61,000 deaths per year. Activation of the immune system against infection M.Tuberculosis is mediated by an inflammatory cellular response that hypothetically has a side effect of the induction and progression of atherosclerosis, and subsequently triggers the plaque rupture. In addition, TB coinfection hypothetically has a relationship with the clinical course of patients with Acute Coronary Syndrome (ACS). This study aimed to examine the short-term outcomes of ACS patients with the comorbid of pulmonary TB.

Methods: This is a prospective cohort study included 40 ACS subjects with or without pulmonary TB comorbidities. The collection of clinical, laboratory, and radiological data was carried out at the beginning of the study, and followed up for up to 30 days to measure the primary outcomes: the length of stay (LOS) and mortality.

Results: A total of 22 subjects (55%) had comorbid of pulmonary TB, and this group had a longer mean LOS duration compared to subjects without pulmonary TB (6.8 vs. 5.7 days, p = 0.045). There was no difference in mortality between the two groups. Subjects with comorbid of pulmonary TB also had higher CRP values compared to non-TB group (54.2 vs. 9.4, p = 0.049). Pulmonary TB comorbidity correlated with LOS duration (r = 0.33, Relative Risk 1.8, p = 0.033)

Conclusion: Pulmonary TB comorbidity has a significant correlation with the prolonged LOS duration in ACS patients, but not with the short-term mortality.
