Abstract (Abstrak)

Introduction: Colorectal cancer (CRC) is the third most common cancer and fourth leading cause of cancer-related death worldwide. CRC are at a very high risk of becoming malnourished. Preventing malnutrition through early detection is important to reduce prevalence of malnutrition, mortality, and cost of treatment. The adequate management of CRC requires a deep knowledge of the role played by the clinical factors involved in the disease pathogenesis and can help to identify prognosis biomarkers. Some biomarkers have been investigated and studies in this field have reported that tumor progression and cancer prognosis can be determined not only by tumor characteristics but also by nutritional and immunological status. The neutrophil-lymphocyte ratio (NLR) is often used as an early screening method as it is correlated with poorer survival in CRC. Neutrophil-lymphocyte ratio (NLR) and nutritional status may provide a prognostic value in colorectal cancer (CRC).

Aims: To evaluate the relationship between nutritional status, length of stay (LOS), and clinical outcomes to NLR in patients with CRC in Dr. Wahidin Sudirohusodo Hospital, Makassar, Indonesia.

Method: clinical data of 106 subjects were collected from hospital’s medical record in a cross-sectional study method between May – June 2021. Nutritional status was graded with Subjective Global Assessment (SGA) method. High NLR has a value >3, and low NLR ≤3. We evaluated the nutritional status based on subjective global assessment (SGA), we assessed the inflammatory response and relationship length of stay and clinical outcomes based on neutrophil to lymphocyte ratio (NLR). Data were analysed using SPSS version 25.0
Results: Deceased patients had significantly higher NLR value (16.68±14.06 vs 10.74±12.2, p<0.05), higher lymphocyte value (7.01±9.47 vs 2.26±3.66 cells/mm3), and older age (57.3±12.6 vs 52.97±10.14 years old). NLR value are not associated with nutritional status (p>0.05) and LOS (p>0.05). SGA is the strongest independent variable that could influence clinical outcome (B = 0.812, p>0.05, OR (95% CI) = 2.253 (0.944 – 5.377)).

Conclusion: NLR value could be used to predict clinical outcomes in patients with CRC.
