Analisis Ekspresi Gen Matrix Metalloproteinase(MMP)-8 dan Tissue Inhibitor of Metalloproteinase (TIMP)-1 padaAdhesi Intraperitoneal Setelah Pemberian Asam Hialuronat-Carboxymethylcellulose (HA-CMC) atau Virgin Coconut Oil (VCO).(Dibimbing oleh Andi Asadul Islam,Muh. Nasrum Massi,Ibrahim Labeda)Tujuan :Untuk mengetahui ekspresi mRNA gen matrix metalloproteinase(MMP)-8 dan tissue inhibitors of metalloproteinase (TIMP)-1 peritoneumnormal dan jaringan adhesi intraperitoneal setelah pemberian virgin coconutoil (VCO) dan asam hialuronat-carboxymethylcellulose (HA-CMC).Metode :Penelitian ini dilakukan pada hewan coba rattus norvegicus strainwistar yang dibagi dalam 3 kelompok. Kelompok pertama adalah kelompokkontrol, kelompok kedua diberikan VCO dan kelompok ketiga dengan HACMC.Dua minggu kemudian dilakukan relaparotomi (post) untuk mengambiljaringan adhesi dan dilakukan pemeriksaan ekspresi mRNA gen MMP-8 danTIMP-1. Juga dinilai gambaran makroskopis dan mikroskopis jaringan adhesi.Hasil :Terjadi penurunan bermakna ekspresi mRNA gen MMP-8 jaringanperitoneum pre dan post pada kelompok kontrol (p<0,001). Peningkatanbermakna ekspresi mRNA gen TIMP-1 pre dan post kelompok kontrol(p<0,001). Baik pada kelompok VCO maupun HA-CMC terjadi peningkatantidak bermakna ekspresi mRNA gen MMP-8 (p=0,075 dan p=0,791)danpenurunan bermakna pada ekspresi mRNA gen TIMP-1 untuk kelompok VCO(p=0,015) dan tidak bermakna untuk kelompok HA-CMC (p=0,368). Ujiperubahan ekspresi mRNA gen MMP-8 dan TIMP-1 pada ketiga kelompokadalah berbeda bermakna (p=0,020, p=0,008). Uji statistik di tiga kelompokterhadap gambaran makroskopis dan mikroskopis, menunjukan berbeda tidakbermakna. Uji Spearman untuk hubungan ekspresi mRNA gen MMP-8 danTIMP-1 dengan derajat adhesi; terdapat hubungan bermakna antarapeningkatan TIMP-1 derajat mikroskopis adhesi (p=0,021) di kelompokkontrol. Hubungan bermakna antara peningkatan ekspresi mRNA gen MMP-8dengan derajat mikroskopis (p=0,029) dan hubungan bermakna antarapenurunan ekspresi mRNA gen TIMP-1 dengan derajat makroskopis(p=0,020), juga terdapat hubungan bermakna antara peningkatan ekspresimRNA gen MMP-8 dan penurunan ekspresi mRNA gen TIMP-1 denganderajat makroskopis adhesi (p=0,20).Kesimpulan :Penurunan ekspresi mRNA gen MMP-8 dan peningkatanekspresi mRNA gen TIMP-1 kelompok kontrol menggambarkan tingginyarisiko terjadi adhesi., Hasil ekspresi mRNA gen MMP-8 dan mRNA genTIMP-1 di ketiga kelompok didapatkan berbeda bermakna secara statistik.Peningkatan ekspresi mRNA gen berpengaruh terhadap peningkatan risikoadhesi secara mikroskopis di kelompok kontrol. Di kelompok VCO,peningkatan ekspresi mRNA gen MMP-8 dan penurunan TIMP-1berpengaruh terhadap penurunan risiko adhesi di mikroskopis danmakroskopis, juga di kelompok HA-CMC terjadi penurunan risiko karenapeningkatan ekspresi MRNA gen MMP-8 dan penurunan ekspresi mRNA genTIMP-1Kata kunci :Adhesi intraperitoneal, mRNA gen MMP-8, mRNA gen TIMP-1,VCO, HA-CMC, makroskopis, mikroskopis

Nasrum Massi, Ibrahim Labeda)Purpose : To understand the expression of matrix metalloproteinase (MMP)-8mRNA gene and tissue inhibitors of metalloproteinase (TIMP)-1 in normalperitoneum tissue and intraperitoneal adhesion tissue after the usage ofvirgin coconut oil (VCO) and hyaluronate acid-carboxymethylcellulose (HACMC)Methods : The research is using an experimental lab rat “rattus norvegicusstrain wistar” which is divided into 3 groups. Laparotomy (pre) was done togain parietal peritoneum tissue at the lower left quadrant of the abdomen, inorder to obtain the expression of mRNA gene MMP-8 and TIMP-1. First groupwas control group, second group was treated with VCO, while the third groupwas treated with HA-CMC. After two weeks period, relaparotomy (post) washeld to obtain the adhesion tissue and to examine the MMP-8 and TIMP-1mRNA gene expression.Results : There is a meaningful decrease for the MMP-8 and a meaningfulover expression of TIMP-1 mRNA gene expression in the peritoneum tissuefor the control group at the pre and post laparotomy (p<0,001). While on theother hand, at the VCO nor HA-CMC group, there is an insignificant increaseof MMP-8 (p=0,075 and p=0,791) and significant decrease of TIMP-1 mRNAgene expression for VCO group but insignificant for HA-CMC group (p=0,015and p=0,368). At further study, there are statistically meaningful differences inthe results of MMP-8 and TIMP-1 mRNA gene expression change in threegroups (p=0,020,p=0,008), while macroscopic and microscopic views in threegroups showed meaningless differences. Using Spearman test to check therelation between MMP-8 and TIMP-1 mRNA gene expression with theadhesion degree, resulted in a meaningful relation between increasing TIMP-1 to microscopic adhesion degree (p=0,021) in control group. There is also ameaningful relation between increasing MMP-8 to microscopic adhesiondegree (p=0,029) and also between decreasing TIMP-1 to macroscopicadhesion degree (p=0,020). A meaningful relation is also found betweenincreasing MMP-8 and decreasing TIMP-1 mRNA gene expression tomacroscopic adhesion degree (p=0,20)Conclusion : The decreasing of MMP-8 and the increasing of TIMP-1 mRNAgene expression of control group shows a high risk of adhesion incidence.The results of MMP-8 and TIMP-1 mRNA gene expression of three groupsare statistically meaningful. The increasing mRNA gene expression is going toeffect increasing adhesion risk microscopically in control group. At VCO group,increasing MMP-8 and decreasing TIMP-1 mRNA gene expression is going toeffect the decreasing adhesion risk microscopic and macroscopically.
